4. NSO copy of Birth Certificate (original and photocopy)
5. Medical and dental certificate
6. Certificate of Good Moral Character from the last school attended (original and photocopy)
7. Validation of credits earned (ASCOT form)
8. ID picture (2 copies 2″x2″ and 2 copies 1″x1″)
