– Grade 10 Report Card (F138 with LRN)
– Certificate of Completion (JHS)
– Certificate of Good Moral Character
– NCAE Result (for STEM Only)
– PSA/NSO Copy of Birth Certificate (Original Copy)
– ESC Certification Letter from Junior High School Principal (ESC Grantees Only from Private School)
– QVR Certificate (Qualified Voucher Applicants from Private School Only)
– 2×2 Picture (2pcs)

Grade 12
– Accomplished Clearance
– Grade 11 Report Card
